There are two ways to send in your registration form:

1. Print this PDF as a hard copy, then fill it out and mail it in an envelope with your payment using the mailing address on the form.
2. Fill in the editable form fields within the PDF, then re-save the form (using your name as the file name) and email it to us as an attachment. Payment should be made at least a week before the match starts unless other arrangements have been made (if there are more than 40 wanting to shoot, reserved slots which are unpaid a week before the shoot are subject to be filled from the waiting list).
